Request an Appointment

The first step toward a straighter smile is a consultation, and at Smithtown Orthodontics that consultation is free. Fill in the form below and we will call you back to arrange a time, or call the office directly at (631) 265-3666.

Please note: this form requests an appointment. To cancel or reschedule an existing appointment, or if you need attention today, call the office rather than using the form.

What happens next

We call you back to confirm a time. At the appointment, Dr. Shank examines the teeth himself, tells you whether treatment is needed and when, explains the options, and gives you the fee in writing. Nothing is fitted on the day and nobody will ask you to decide anything on the spot. Full detail is on the first visit page.

To save time, complete your patient forms beforehand and bring your insurance card.

Too busy to come in yet?

You can start with photographs instead. Send us five photos of your smile taken at home and Dr. Shank will review them and tell you whether you are likely to be a candidate for treatment, before you book anything.

It suits people weighing up whether to start, existing patients checking in between visits, and former patients wanting a retainer checked. Ask about it when you call, or mention it in the form.
